Sallie Mae Fisher Health History
Ms. Fisher is an 82-year-old female with a history of chronic congestive heart failure (CHF), atrial fibrillation, and hypertension. During the last 6 months, she has been hospitalized four times for exacerbation of her CHF. She was discharged home last Saturday from the hospital after a 3-day stay to treat increased dyspnea, an 8-pound weight gain, and chest pain.
Ms. Fisher is recently widowed and lives alone. She has a daughter, Thelma Jean, who lives in town but works full time and has family issues of her own. Therefore, family support is limited.
Hospital Discharge Instructions
• Mountain Top Home Health to evaluate cardio-pulmonary status, medication management, and home safety.
• Medical Equipment Company to deliver oxygen concentrator and instruct patient in use. O2 at 2 liters per nasal prongs PRN.

Document the source of the cost (Website address, catalog page cut, name and phone
number of vendor that you talked to, etc.). The TranSystems “Rules of Thumb” online
resource may have costs for your equipment.
Section 6. References.
List at least three references, not including our text – as outlined below.
See “Citing Equipment, References and Brochures” on Isidore for examples of the format
for how the references must be cited.
 One of the references must be a copy of a current (2014 or later) article from a
magazine or journal dealing with specific applications of the equipment. For
example, the article may cover a case study of a company that has installed this
equipment. Discuss the article in your reports.
 The second reference must be an equipment brochure obtained from a
manufacturer for your specific equipment. The brochure and other information
must be obtained from a vendor of the equipment.
 The third reference may be a book or text (but not the course text), journal
article, brochure, Internet article, etc.
 Advertisements for the equipment and newspaper articles are not acceptable as
references.
Section 7. Appendix.
Hard copies of the references must be attached.
